Must serve in a HPSA or MUA for 3 years. California HCAI (Department of Health Care Access and Information) administers the program. Strong preference for FQHC placements. California requires employers to demonstrate inability to recruit a US physician for 6+ months. HPSA score of 14+ recommended for competitive applications. State requires full California medical license at time of application.
Application Tips
California is highly competitive but did NOT fill all 30 slots in FY2024 — slots may remain after the October window. FQHC and community health center placements have significantly better approval rates. Central Valley (Fresno, Bakersfield, Stockton) and rural Northern California (Redding, Eureka) have highest need. Inland Empire (San Bernardino, Riverside) is growing rapidly. Avoid urban LA or SF Bay Area — extremely competitive. Both primary care and specialists now apply in the same October window (recent HCAI change). Budget for high cost of living.
