Western Maryland (Allegany, Garrett counties) qualifies for ARC waivers with unlimited slots. Eastern Shore has growing rural need. Suburban MD near DC has very few HPSA sites. Strong academic infrastructure: Johns Hopkins, University of Maryland, Uniformed Services University. Maryland has relatively high state income tax (graduated 2-5.75% plus county surcharges). H-1B cap-exempt employers include Hopkins, UMD Medical System, NIH.
