Must serve in a HPSA or MUA for 3 years. Texas DSHS administers the Conrad 30 program. Texas has a flex option in policy (up to 10 flex slots) but DSHS has not activated flex eligibility for FY2025/FY2026 — effectively 0 flex in practice currently. No state income tax. $3,000 application fee (refundable if not recommended). Full unrestricted TX medical license required.
Application Tips
Texas is one of the most competitive waiver states but did NOT fill all 30 slots in FY2024 (contrary to common belief). Standard slots fill quickly in October but some may remain. The flex option exists in Texas policy but has not been activated recently. No state income tax. Rio Grande Valley, West Texas (El Paso, Lubbock, Midland-Odessa), and East Texas (Tyler, Longview, Texarkana) have highest HPSA scores. Also consider HHS Clinical Care Waiver (unlimited slots) as alternative. $3,000 fee. Start employer search 8-10 months early.
